2017-2018 SPORTS SEASON
Neptune High School Sports Physicals will be conducted
In the Nurse's Office After School on June 13th and 14th
Deadline for NHS physical paperwork submission is June 8th, 2017
Neptune Middle School Sports Physicals will be conducted
in the Nurse's Office After School on June 6th and 12th
Deadline for NMS physical paperwork submission is May 31st, 2017
IMPORTANT INFO FOR ALL ATHLETES:
PARENTS YOU MUST SIGN THE COVER PAGE AT THE BOTTOM FOR CONSENT.
Please submit all completed questionnaires including
all seven (7) pages in order to have a physical in school.
You may keep the Sudden Cardiac Death Pamphlet for your reference
If you miss the sports physicals during the school day you may contact the:

You must arrive with your forms and a parent must accompany you or
you must have the cover letter signed by a parent.*********************

Monday through Friday – 8:30 a.m. to 4: 30 p.m.

Please note all students who have a current sports physical on file are required to have a Health History Update form completed along with a Sudden Cardiac Death Sign Off Sheet. The pamphlet is under forms of the Athletic webpage. If a student cannot attend the school physicals, he/she can still receive a FREE physical by appointment only at the K. Hovnanian Children’s Hospital Wellness Center at Midtown Community Elementary School. Please review the forms page under Athletics for all sport physical exam forms. You must drop them off the school nurse’s office in your child’s school.(To be completed and handed in to the nurse prior to each season)
